For Per Claims Basics 

• Per Claim Basics is a usefully plan for temporary work. Where you don’t need stay with us longer but need work done on the claims.
• Software or PMS (practice management software) Cost is not not included Physician has to pay that cost which they are already paying. 
• Any other cost that incurs from Physician office side is Physician’s Responsibility. Our Side Cost is Our Responsibility (Fax, Phone, PC, administrative cost of our side, Technical Cost. Training Cost of Our Employees. Our Employee Salary etc).
• Includes All Features of Medical Billing and Coding. Credentialing & Patient Collection & Patient Billing not included in the % plan.
• This Plan is for All Claims (Fresh Claims) that need billing by us and are less than 500 Claims. So We receive Mix of Fresh claims. This Plan is not for Physician looking for A/R follow-up only or OLD A/R Clearing.

• Minimum you pay for this plan is 100$ per month, even if you don’t have any billing in that month (0 claims due to any reason). But if you have more than 50+ Claims you will be charged for ( number of claims x per dollar Charge). Under 50 Claims you will pay 100$ in that month. Minimum cost is charged due to Administrative cost per month only.
• You Pay Monthly Charges after end of each month calculation of number of claims.
• Setup Cost is 100$ for this plan (non refundable/ One Time Cost / Paid in Advance).
• E.g if you have 65 Claim in first month, and per claim cost is 2$.

For 1st Month
You pay (100$ one time Setup Cost in advance and after one month you pay 65 x 2$ = 130$).

From 2nd Month onwards
For the next month you have same 65 claims you pay 65 x 2$ =130$ only.
